Thank you for your interest in joining one of our volunteer or leadership teams at NewLife Church! We understand that this application is a bit detailed; however, we believe it is important to do our best to shepherd and protect the church body.

Our desire is for every leader to be equipped, healthy, and supported in their area of service. None of us are perfect, but we are committed to living faithful lives that are pleasing to the Lord as we serve Him and His people.

There are several opportunities to serve in a volunteer position here at NewLife. You will have a chance to select the area in which you are most interested. Why this form matters?

At NewLife Church, we believe that leadership and serving are not about positions or titles—they are about servanthood, character, and spiritual health. As we grow, it is vital that those who lead and serve others are first deeply rooted in their relationship with Christ, living out their faith authentically in every area of life. (Notice we didn't say perfectly!)

This form is designed to help us walk alongside you in that process. The questions may seem detailed, but each one serves a purpose: to help you reflect on your spiritual journey and to ensure that those stepping into leadership are healthy, growing disciples. We focus on four key areas:
  • Personal Relationship with Christ: Loving Jesus deeply and pursuing Him daily through the Word, personal worship, and prayer.
  • Home Life: Living out faith authentically with those closest to you (including singleness, marriage, parenting, and finances).
  • Church Life: Serving the Body of Christ with humility, unity, and joy through community, giving, and corporate worship.
  • Life in the World: Reflecting God’s love and making disciples wherever you go.

As Paul wrote in Ephesians 4:11–13, God calls leaders to “equip the saints for the work of ministry, for building up the body of Christ...” This responsibility rests on the pastors, elders, and current leadership of this church. Our goal is not perfection—it’s growth. We want to help you flourish in your walk with Jesus and your ability to help others do the same.

Struggling in a certain area does not automatically disqualify you from leading or facilitating; rather, your honesty helps us support you effectively. Please select the answers that most accurately represent you. Use this as an opportunity to reflect on your victories and identify where you would like to grow.

Remember, this is simply an interest form for our church leadership to get to know you and your heart for leadership and serving. Once submitted, someone from our team will reach out to discuss the details, explain how you can take the next steps, and possibly give you the opportunity to commit to a volunteer leadership or serving role here at NewLife Church.

Next Steps

Once we review this request and approve, a leader will contact you. Some areas of leadership / service requires additional information which they will send to you.  

For example: If you're pursuing to become a Living on Target facilitator, you will receive a form that will ask more information like....
-What day of the week you plan to meet?
-Frequency of meetings
-Start and End time of meetings
-Specific age group you want to target
-Max number of participants you can hold in your space, etc.


Be looking for an additional email from our church leadership. 

Lastly, for certain volunteer positions we ask for character references. 

If you do not see options to add references below, then your position does not require references. Proceed to "submit".
